How to Treat Back Acne

Back acne is harder to treat than face acne — different pH, more friction, more sweat. The protocol below is what dermatologists actually prescribe.

Body wash with 2% salicylic acid daily. Glycolic acid body lotion 3× weekly. Loose breathable fabrics. Shower immediately after sweating. Spironolactone if hormonal.

The full method.

  1. Use a salicylic acid body wash. CeraVe SA Smoothing Cleanser or Neutrogena Body Clear. Daily. The most-effective single step.
  2. Apply glycolic acid body lotion 3× weekly. Mandelic Acid 5% body lotion or Eucerin Smoothing Body Lotion. Resurfaces skin, prevents new clogging.
  3. Wear loose, breathable fabrics. Tight synthetic clothing traps sweat and bacteria. Cotton + loose-fit reduces flares.
  4. Shower immediately after sweat. Sweat + sebum on skin = bacteria growth. Quick shower or wipe-down post-workout is non-negotiable.
  5. Wash bedding weekly. Pillowcases, sheets — gather oils that re-deposit on the back.
  6. See a doctor about spironolactone. If acne is hormonal (chin, jaw, back, peri-period), spironolactone is the most-effective treatment. Prescription-only.
  7. Avoid heavy hair products on shoulders. Conditioner, hair oil, leave-in — all migrate to back during showering. Wash hair thoroughly.
